Terms and Conditions:


This website is provided by Set in Motion Physiotherapy. By using this website you agree to be bound by the terms and conditions set forth in the User Agreement. Set in Motion Physiotherapy has the right to change these terms and conditions from time to time without notice to you.

1) Privacy:

  • For the privacy policy of Set in Motion Physiotherapy please visit our home page to find information regarding this policy.

  • You will be examined & treated on a physiotherapy plinth in the clinic in a private room with the door closed. The Physiotherapist will provide the option of a gown for privacy. If you feel uncomfortable or concerned in any way, you acknowledge you will let the physiotherapist know.

    The physiotherapist needs to examine the area of injury and any associated areas by:

  • Asking you to remove appropriate clothing to allow examination of the area &/or to wear a gown. If you do not understand, feel uncomfortable or concerned in any way, you acknowledge you will let the physiotherapist know.

  • Touching the area of injury + other areas which may be related (occasionally under clothing if appropriate). If you do not understand, feel uncomfortable or concerned in any way, you acknowledge you will let the physiotherapist know.

2) Treatment:

  • It is important to us that you understand the management & treatment plan. You may be asked to perform exercise using bands, weights and exercise equipment. If you do not understand the treatment, feel uncomfortable or concerned in any way, you acknowledge you will let the physiotherapist know.

  • Set in Motion Physiotherapy cannot guarantee results to our patients. Set in Motion Physiotherapy Employee’s will aim to provide evidence based practice to help our patients in a safe manner.

  • This may include massage, spinal mobilisation/manipulation or joint mobilisation - the therapist will explain the treatment and any risks involved at the time of treatment. If you do not understand the treatment, feel uncomfortable or concerned in any way, you acknowledge you will let the physiotherapist know. 

3) Your physiotherapist will always endeavour to explain:

  • Why they are examining injured & associated areas & the most likely diagnosis

  • The consequences and risks of proposed treatment

  • The probable consequences of not undertaking the treatment

  • Any alternative treatment or courses of action that may be considered

  • Ensure you have the opportunity to ask any questions

If you do not understand, feel uncomfortable or concerned in any way, you acknowledge you will let the physiotherapist know.


4) Confidentiality:

  • You give permission for your physiotherapist to discuss appropriately my injury management in confidence with other physiotherapists, doctors or allied health professionals in order to benefit my injury management.

5) Consent:

  • You acknowledge that at any point you have the right to withdraw consent for assessment and/or treatment. You understand there are risks involved with assessment and treatment. You understand by signing this form you are providing consent to physiotherapy assessment and treatment.

6) Warranty of consumables:

  • Any consumable provided is warranted for replacement or repair at no cost should a failure occur under certain conditions. This does not include recurrent wear due to poor compliance, failure due to accident or injury, failure due to loss, or negligence on your behalf.

7) Attendance of appointments:

  • In the event that an appointment requires to be rescheduled, then 24 hours notice must be provided or a fee may be charged to cover the cost of lost clinical time.

  • Whilst contact is made prior to the appointment, if a call or message is not received this does not constitute grounds for non-attendance.

  • If you are late to an appointment, this will result in a shorter consultation time.

  • If you arrive late or do not attend an appointment you may be charged a cancellation fee.

8) Payment

  • EFTPOS payment at the time of service delivery

  • We accept bank transfer in special circumstances

  • Unpaid fees may result in further action being taken by Set in Motion Physiotherapy. This may involve a claim being made against the client and or patient in the small claims tribunal.

  • If you sign up to a Set in Motion Physiotherapy program (‘ACL knee Program’, ‘Pilates and Back Program’, ‘Hip and Knee Program’), you agree to pay in full prior to commencing the program.

9) Cancellation fees and charges:

The policy includes initial and standard physiotherapy appointments:

  • You cancel within 24 hours prior to the scheduled appointment OR you do not show for your scheduled appointment. A fee of 50% will be charged to the patient if either criteria is met.

  • Notice of cancellation should be made to the following email address: admin@simphysio.com.au or calling 9098 91

    The policy includes Set in Motion Physiotherapy programs (‘ACL knee Program’, ‘Pilates and Back Program’, ‘Hip and Knee Program):

  • If a patient gives 24 hours notice of not attending the group class then that appointment can be moved to a seperate group class.

  • If a patient does not give 24 hours notice of not attending a class then the patient is unable to transfer that class to a later date.

10) Attendance of appointments:

  • Set in Motion Physiotherapy Programs including ‘ACL knee Program’, ‘Pilates and Back Program’ ‘Hip and Knee Program: If a patient gives 24 hours notice of not attending the group class then that appointment can be moved to a seperate group class.

  • All group class appointments must be used within 6 months of the initial program assessment.

11) Responsibilities of SIM Physiotherapy staff:

  • Set in Motion Physiotherapy staff have a responsibility to provide you with all information regarding your care

  • Treat you with respect

  • Advise you of your treatment plan and estimates fee’s

  • Protect your privacy and confidential information

  • Keep accurate records

  • Issue invoices and records of payments

  • Provide appropriate care and abide by the standards set out by AHPRA

12) Responsibilities of the client/patient (You):

  • You agree to pay for services provided on the day of service

  • You agree to treat the staff of Set in Motion Physiotherapy with respect

  • You agree to communicate to the staff of Set in Motion Physiotherapy about any concerns or questions they may have about the service and or treatment being prescribed

  • You understand any risks in undertaking Physiotherapy treatment and/or an exercise program

  • You agree to Set in Motion Physiotherapy’s cancellation policy

  • You agree to pay in full of any Set in Motion Physiotherapy Program: ACL knee Program, Pilates and Back Program, Hip and Knee Program

13) Complaints:

  • If the client and or patient is not satisfied with the service being prescribed they should speak with our admin staff. The complaint will be passed on to our Director who will discuss the complaint further.

  • Feedback can be provided directly to admin@simphysio.com.au and a staff member will contact the patient directly to discuss the complaint further.

16) Liability: 

  • You agree Set in Motion Physiotherapy and its employee’s are not liable for the following:

  • Death or personal injury or illness to you or a recipient arriving at, attending or leaving the clinic

  • Any loss or damage that you or a recipient may suffer due to any errors or omissions on any form we or our employees have provided

  • Any damage to any personal property belonging to you or a recipient, except where such loss is caused by our employees’ gross negligence or wilful default

  • Any economic losses, including loss of revenues, profits, contracts, business or anticipated savings, loss of goodwill or reputation, any other special losses suffered by you or a recipient where that loss is incurred directly or indirectly as a result of our provision of services to a recipien

14) Photography and Video:

  • The Client acknowledges that Set in Motion Physiotherapy may take photo’s, video’s and voice recording during treatment. The client has the right to give or take away consent. Please ensure you email admin@simphysio.com.au should you wish to withdraw consent.

15) Waiver:

  • I acknowledge attending Physiotherapy can carry a significant risk of physical harm or personal injury, including permanent disability and/or death.

  • I agree that Set in Motion Physiotherapy and its employees will not be liable for any injury, loss, damage, death or economic loss..

  • I agree that Set in Motion Physiotherapy and its employees will not accept responsibility for loss or damage to any personal belongings left unattended on the premises.
